Download the necessary forms, complete them and attach them to the submission form at the bottom of this page.

For permits that require fees, payment can be made by mailing a check to PO Box 418A, Medora, ND, dropping off a check at the City Office (465 Pacific Ave), or by credit card over the phone at 701-623-4828.
